Indirect Collection:

We sometimes receive health information about you from other providers – including hospitals, specialists, laboratories and community health services – to support your ongoing care.

 

Purpose:

Information received from other providers is used to provide you with safe and effective health care, coordinate your care across services, and to meet our reporting obligations.

 

Who received it:

Information may be shared with other members of your care team, and with THINK Hauora for programme administration and reporting to Health New Zealand as required.

 

Your rights:
